Instructions for Completing the Annual Withholding Reconciliation Statement (REV-1667) 

A W-2, Wage and Tax Statement for each employee must be submitted annually on or before January 31 following the year in which wages were paid to be considered timely. Any W-2 or 1099 information submitted through a file upload will populate in Part I of the records filed through e-TIDES. Follow the Steps below to manually enter or edit any W-2 or 1099 information.

Step 1 - Main Menu

Select 'File Return/Payment' from the left navigation menu. Then select Employer Withholding Tax from the Select Tax System drop-down box and click the Next button. 

Step 2 - Please Select Account Screen

Select File an Employer Withholding Tax Return or Payment. Then select the appropriate enterprise you wish to file for from the drop-down box and click the Select button.  

Step 3 - Filing Options Screen

Select W-2/1099 (Transmittal) Annual Withholding Reconciliation Statement and select the appropriate period you wish to file for from the drop-down box, and click the Continue button. The current period due in the drop-down box will display.  

If you wish to file for a period other than what is displayed select File/Pay Different Periods and click the Continue button. You will remain on the same screen and the drop-down box displays all periods allowed to be filed. 

Step 4 - Employer W-2/1099 Menu Screen

You will be presented with the following options:

Make the appropriate selection and click the Continue button.  

Step 5a - Enter/Edit W-2 Employee Wage Records

If you select to Enter W-2 Employee Wage Records from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the W-2 Employee Wage and Tax Statement Screen.  

Enter in the required information from the employee's W-2. If you need to add additional employees, click the Add Next Employee button.  Your previous employee data will be saved and you can then enter the next employee data. 

You may stop at any time and resume entering in the employee information. If you decide to stop and begin again later, select Enter/Edit W-2 Employee Wage Records from the Employer W-2/1099 Menu screen.

When you have entered all of your employee information, click the Return button. You will then be taken back to the Employer W-2/1099 Menu screen.

Step 5b - Enter/Edit W-2 Employee Wage Records

If you select to Edit W-2 Employee Wage Records from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the W-2 Employee Wage and Tax Statement Screen. This option is used when you have entered employee information and stopped before finishing.

You will be given a list of the employee W-2 information you have already entered and have the ability to sort your list by Employee Number, SSN, Name, Address, State Wages or State Income Tax. You also can view from 10 employees to 50 employees per page.

If all of the information is correct and you are finished entering employee data, click the Return button. If you need to edit an employee's information, select the Radio button next to the employee record you wish to change, then click the Edit button.

Step 5c - Enter/Edit 1099-R Employer Distributions

If you select to Enter 1099-R Employer Distributions from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the 1099-R Employer Distributions form screen.  

Enter in the required information from the employee's 1099-R.  If you need to add additional employees, click the Add Next Employee button. Your previous employee data will be saved and you can then enter the next employee data. 

You may stop at any time and resume entering the employee information. If you decide to stop and begin again later, select Enter/Edit 1099-R Employer Distributions from the Employer W-2/1099 Menu screen.

When you have entered all of your employee information, click the Return button.  You will then be taken back to the Employer W-2/1099 Menu screen.

Step 5d - Enter/Edit 1099-R Employer Distributions

If you select to Edit 1099-R Distributions from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the 1099-R Distributions form screen. This option is used when you have entered employee information and stopped before finishing.

You will be given a list of the employee 1099-R information you have already entered and have the ability to sort your list by Employee Number, SSN, Name, Address, State Wages or State Income Tax, Gross Distributions, Distribution Code or State Distribution. You also can view from 10 employees to 50 employees per page.

If all of the information is correct and you are finished entering employee data, click the Return button. If you need to edit an employee's information, select the Radio button next to the employee record you wish to change and click the Edit button.

Step 5e - Enter/Edit 1099-MISC Income

If you select to Enter 1099-MISC Income from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the 1099-MISC Income form screen.  

Enter in the required information from the employee's 1099-MISC.  If you need to add additional employees, click the Add Next Employee button. Your previous employee data will be saved and you can then enter the next employee data. 

You may stop at any time and resume entering the employee information. If you decide to stop and begin again later, select Enter/Edit 1099-MISC Income from the Employer W-2/1099 Menu screen.

When you have entered all of your employee information, click the Return button.  You will then be taken back to the Employer W-2/1099 Menu screen.

Step 5f - Enter/Edit 1099-MISC Income

If you select to Edit 1099-MISC Income from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the 1099-MISC Income form screen. This option is used when you have entered employee information and stopped before finishing.

You will be given a list of the employee 1099-MISC information you have already entered and have the ability to sort your list by Employee Number, SSN, Name, Address, State Wages or State Income Tax, Gross Distributions, Distribution Code or State Distribution. You also can view from 10 employees to 50 employees per page.

If all of the information is correct and you are finished entering employee data, click the Return button. If you need to edit an employee's information, select the Radio button next to the employee record you wish to change and click the Edit button.

Step 5g - Enter REV-1667 (Transmittal) Employer Withholding Tax W-2 Annual Withholding Reconciliation Statement

If you select to Enter REV-1667 (Transmittal) Employer Withholding Tax W-2 Annual Withholding Reconciliation Statement from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the REV-1667 (Transmittal) Employer Withholding Tax W-2 Annual Withholding Reconciliation Statement form.

Part I - Reconciliation will be pre-populated with the information you entered for your employees through the Enter/Edit W-2 Employee Wage Records, Enter/Edit 1099-R Employer Distributions and/or Enter/Edit 1099-MISC Income options or through a file upload. If you transmitted W-2, 1099-R and 1099-MISC forms to the Commonwealth through another method other than e-TIDES, you must adjust these figures.

Part II - Annual Reconciliation will be pre-populated with the W-3 Quarterly Reconciliation form. Please note that Part II of the W-2 Annual Withholding Reconciliation Statement will only reflect W-3 returns filed through e-TIDES.  

Click the Continue button when the W-2 Annual Withholding Reconciliation Statement is complete. You will then be taken to a Verification screen. If the information is correct, click the Submit button. If the information is incorrect, click the Edit button to make the necessary corrections.

  Step 5h - Enter/Edit REV-1667 (Transmittal) Employer Withholding Tax Annual Withholding Reconciliation Statement

If you select to Edit REV-1667 (Transmittal) Employer Withholding Tax Annual Withholding Reconciliation Statement from the Employer W-2/1099 Menu screen and click the Continue button, you will be taken to the REV-1667 (Transmittal) Employer Withholding Tax Annual Withholding Reconciliation Statement form.

Part I - Reconciliation will be pre-populated with the information you entered for your employees through the Enter/Edit W-2 Employee Wage Records, Enter/Edit 1099-R Employer Distributions and/or Enter/Edit 1099-MISC Income options or through a file upload. Note: Line 3, PA Income Tax Withheld, will reflect the total of W-2s, 1099-Rs and 1099-MISCs originally entered. If you need to edit this information, you must overwrite the information currently provided in the appropriate field.

Part II - Annual Reconciliation will be pre-populated with the W-3 Quarterly Reconciliation form. Please note that Part II of the W-2 Annual Withholding Reconciliation Statement will only reflect W-3 returns filed through e-TIDES.  If you wish to edit this information, please make the appropriate changes to either the compensation or tax withheld columns before continuing.

Click the Continue button when the W-2 Annual Withholding Reconciliation Statement is complete. You will then be taken to a Verification screen. If the information is correct, click the Submit button. If the information is incorrect, click the Edit button to make the necessary corrections.
